CTBN Online Application

Before completing the application form, you must read and agree with:

The Mother-Friendly Childbirth Initiative (MFCI) by the CIMS, and
The Six Care Practices that Support Normal Birth, by the Lamaze Institute.

To join, complete the following steps:

Read the Mother-Friendly Childbirth Initiative online and complete the online
endorsement form.
Complete the Online Application that follows.
Submit payment by credit card by following this payment link, or make your check
payable to the Central Texas Birth Network and mail to address below.
If you are applying for a Premier Membership, you will need to submit by
email the text you would like for your personalized webpage along with a high
resolution JPEG photo of yourself.

Please allow up to three week for your member application to be approved. We do verify that your name is on CIMS' MFCI endorsement list.

***For corporate memberships, please complete an application for each provider, and cross reference the company name to identify who is participating in that corporate membership.
